Passport
Application
for New Passport:
Use
this application to apply for a passport if:
- You
have never before had an Israeli passport.
- Your
last passport was issued 10 years ago.
Fill
out the application and send it to the Consulate
and enclose:
- Two
color passport style photographs (2" X 2",
facing front)
- Fees
- $50.00 by cash or money order to each passport
requested for an Israeli citizen from the
age of 17. For citizens under 17 years old,
the fee is $17. $ 10 for return mailing charges.
Kindly note above Consular fees are to be
made in cash or money order only. Personal
checks will not be accepted.
- Your
old passport (if you have one), which will
be returned to you.
New
Passport Application form Application for Passport
Renewal:
Use
this application to apply for a passport if
your last passport was issued less than 10 years
ago.
Fill
out the application and send it to the Consulate
and enclose:
- Your
old passport.
- The
cost of the renewal is free. Please send it
registered mail, include $10 for the return
postage. Be sure to include your address and
phone number. Kindly note above Consular fees
are to be made in cash or money order only.
Personal checks will not be accepted.
- A
passport can be renewed until 10 years after
the first issue.
Loss
of Passport
In
addition to the previously mentioned regulations
needed for a new passport, you need the following:
- You
must come to the Consulate in Person - this
procedure cannot be done by mail
- Bring
a photo ID
- Police
report of loss
- Two
color passport style photographs (2" X 2",
facing front)
- The
fee is $4.00 for the declaration form, $50.00
penalty, plus $50.00 for issuing a new passport.
- $10.00
for return by mail.
